The Healthy Living Program (HLP) at Canadian Mental Health Association, Durham was recently recognized for excellence and innovation in healthcare by Accreditation Canada. HLP was seen as a leading practice as it relates to one of the Primary care Standards, specifically: “The clinic works with each client and family to promote health and prevent disease.”
The HLP is innovative in that it is one of the few programs that integrates vascular health and mental health. The program will be formally recognized in February 2013 at the Accreditation Canada Quality Conference.
Accreditation Canada recognizes leading practices across the Canadian care continuum, that are particularly creative with solutions that improve quality in Canadian healthcare delivery.
