Community Care Information Management (CCIM) has a strategic vision to provide seamlessly integrated, community-based client care where all service providers can securely share and access consistent and accurate information electronically. CCIM consists of two streams and a number of projects to support the community care sector.
CCIM was asked by the community care sector to initiate an advisory group to keep the needs and concerns of those receiving community-based health care top-of-mind. In response to this request CCIM has established an Integrated Assessment Record (IAR) Service User Advisory Group.
The IAR is being implemented across multiple health care sectors in all 14 Local Health Integrated Networks to enable the electronic sharing of assessment data. These sectors include community mental health (CMH), addictions, community support services (CSS), long-term care homes (LTCH) and community care access centres (CCAC). In order to ensure that all user needs are addressed, the IAR Service User Advisory Group is made up of a cross section of 11 stakeholders who use or have used services in these sectors.
In order to optimize learnings from the previous efforts of consumer/survivors, membership will include individuals from the Community Mental Health Common Assessment Project Consumer Working Group. The first meeting for the IAR Service User Advisory Group is scheduled for October 2011.
