According to the Ministry of Health and Long-Term Care (MOHLTC), five percent of patients account for two-thirds of healthcare costs. These are often individuals with multiple, complex conditions. When the hospital, the family doctor, the long-term care home, community organizations and others work as a team, the patient receives better, more coordinated care. This is the vision of the 47 Community Health Links across the province. The MOHLTC is hosting a conference, “Ways & Means to make Health Links work,”that will explore the future of Ontario’s Health Links programs.
