The Four Villages Community Health Centre (CHC) offers primary care services in the west end of Toronto, with programs that include treatment, disease prevention, health promotion, and capacity building. It also provides psychiatric consultations as part of its shared care initiative with St. Joseph’s Health Centre and the Centre for Addiction and Mental Health. As part of this initiative, staff psychiatrists from these two hospitals are at the Four Villages CHC for two to three three-hour sessions each month, providing consultations with clients. They also offer capacity-building opportunities for the centre’s team of professionals, including general practitioners, nurses, social workers, and occupational therapists.
